Fase Krisis Blast Chronic Myeloid Leukemia dengan Acute Renal Failure

Leukemia mieloid kronik merupakan penyakit mieloproliferatif dengan komplikasi tersering gagal ginjal. Fase krisis blast cukup jarang tetapi terberat dari seluruh fase leukemia mieloid kronik dengan risiko kematian tertinggi. Kasus: Perempuan 72 tahun dengan leukositosis 218.700/µl dan gagal ginjal akut dengan kreatinin 7,1 mg/dL. Dari morfologi darah tepi disimpulkan leukemia mieloid kronik dengan sel blast 54%. Pasien diterapi dengan dua sesi leukapheresis, dan idarubicin selama 3 hari dengan terapi pengganti ginjal berkelanjutan. Selanjutnya pasien diberi sitarabin IV selama 7 hari, dengan hasil remisi.
Chronic myeloid leukemia is a myeloproliferative disease with renal failure as the most common complication. Blast crisis is the rarest but worst phase of chronic myeloid leukemia with highest mortality. Case: A 72-year-old woman presented with leukocytosis 218,700/µl, and acute renal failure with creatinine 7,1 mg/dL. The peripheral blood morphology showed chronic myeloid leukemia with 54% blast cells. The patient was treated with two sessions of leukapheresis, and idarubicinfor 3 days in the setting of continuous renal replacement therapy, followed with 7 days of continuous cytarabineinfusion. The patient went into a complete remission.
