Abstract

Latar Belakang: Penambahan nitrous oxide akan mempercepat waktu induksi anestesi, karena second gas effect dan concentration effect. Tujuan: Membandingkan kecepatan induksi anestesi sevoflurane 8% dengan atau tanpa nitrous oxide, dengan teknik single breath vital capacity induction berdasarkan nilai Index of Consciousness. Metode: Lima puluh pasien ASA I dan ASA II yang menjalani operasi elektif dengan anestesi umum menggunakan teknik induksi inhalasi secara acak dibagi menjadi 2 kelompok: sevoflurane 8% + nitrous oxide 50% : oksigen 50% (kelompok I) dan sevoflurane 8% + 100% oksigen (kelompok II). Pengukuran interval waktu pada alat Index of Consciousness dari 100 hingga 60. Data dianalisis dengan uji-T dengan derajat kemaknaan p<0,05. Hasil: Karakteristik pasien pada kategori jenis kelamin, umur, berat badan, tinggi badan, indeks massa tubuh, tekanan darah sistolik, tekanan darah diastolik, laju jantung, dan laju pernapasan pada kedua kelompok tidak berbeda bermakna. Waktu untuk perubahan nilai Index of Consciousness dari 100 hingga 60 untuk kelompok sevoflurane 8% + oksigen 50% : nitrous oxide 50% (99,4 ± 20,96 detik) tidak signifikan lebih cepat dibandingkan kelompok sevoflurane 8% + oksigen 100% (108,52 ± 18,87) (p = 0,113). Simpulan: Penambahan nitrous oxide dengan teknik single breath vital capacity tidak meningkatkan waktu induksi anestesi pada sevoflurane 8%.

 

Background: The addition of nitrous oxide increase induction time of anesthesia agent, because of second gas effect and concentration effect. Objective: To compare induction time of 8% sevoflurane with and without nitrous oxide using a single-breath vital capacity induction based on Index of Consciousness. Method: Fifty ASA I and ASA II patients who will undergo elective surgery under general anesthesia using inhalation induction techniques are randomly divided into 2 groups: 8% sevoflurane in 50% nitrous oxide : 50% oxygen (group I) and 8% sevoflurane in 100% oxygen (group II). The time interval showed by Index of Consciousness device is from 100 to 60. Data was analyzed with T- test with significancy p < 0.05. Results: The two groups had similar distribution on sex, age, weight, height, body mass index, systolic blood pressure, diastolic blood pressure, heart rate, and respiratory rate. Time required for Index of Consciousness value change from 100 to 60 in sevoflurane group 8% + 50% nitric oxide : 50% oxygen (99.4 ± 20.96 seconds) was not significantly shorter than in sevoflurane group 8% + oxygen 100% (108.52 ± 18.87) (p = 0.113). Conclusion: The addition of nitrous oxide do not increase induction time of anesthesia with a single-breath of 8% sevoflurane.
